What is the EksoNR exoskeleton?

The EksoNR medical exoskeleton is an upgraded version of the EksoGT, a bionic exoskeleton developed by Ekso Bionics specifically for a wide range of neurological patients. EksoNR is a sophisticated and comprehensive rehabilitation tool that, in addition to gait training itself, enables patients to perform many activities that precede walking, such as shifting body weight from one side to the other, side steps, walking backward, and performing squats within several different ranges of motion.

EksoNR allows users to replicate movement that closely resembles normal gait, both physiologically and biomechanically. Walking in the device is achieved by shifting the patient’s body weight or by initiating a step with the push of a button. Battery-powered motors move the legs, replacing lost neuromuscular functions. In addition, the innovative SmartAssist technology allows for precise adjustment of parameters to the patient’s individual needs throughout the course of therapy.

Indications

EksoNR is an exoskeleton for gait rehabilitation intended for use under the supervision of a physical therapist. It is intended for patients with varying degrees of paralysis (partial, complete) or paresis (flaccid, spastic, hemiplegic), which may result from neurological conditions or injuries (e.g., stroke, spinal cord injury, traumatic brain injury).

Almost every patient cleared for training by a doctor and physical therapist was already walking during the first session. EksoNR is a rehabilitation device designed for both early and chronic gait rehabilitation, in both hospital and outpatient settings.

Smart Assist Software

Operating modes available for the Smart Assist module

Depending on the goal of the training session, the therapist can set a specific force value at which the EksoNR is to assist the patient’s movement, or allow the device to automatically detect the force applied by the patient and adjust the force with which it assists the movement in real time.

Max Assist

The device uses the maximum level of assistance. The patient is not required to use their lower limbs to interact with the device. To initiate movement, the patient must be able to maintain balance and shift weight from one side to the other.

Adaptive Assist

The patient applies maximum force. The device automatically detects and supplements the missing force, resulting in a smooth gait for the patient. The therapist receives feedback on the amount of force the device is applying to assist the patient in taking a step within a specified time frame.

Fixed Assist

Each of the EksoNR’s legs operates at a specific level of assistance (0–100%), allowing the patient to take steps with a specific level of motor support. The assistance levels are determined based on data collected while operating in Adaptive Assist mode, enabling the therapist to monitor the impact of different training modes on the progress of the rehabilitation process.

Choice of movement mode

The EksNR exoskeleton for people with disabilities is fully customized to each patient’s needs. It is equipped with a comprehensive set of attachments that can be used to compensate for differences in limb length or provide additional back support. Mobility aids that support the arms—such as crutches, a walker, a sling, and a walking cane—are available and are used at various stages of the rehabilitation process, depending on the patient’s progress. In addition, the EksoNR is equipped with various walking programs that the clinical team can easily select from the settings menu.

The step generator software has enhanced security.

The Step Generator software helps patients stand up and regain their walking function as quickly as possible. EksoNR calculates and adjusts the path of each step during the swing phase at a rate of 500 times per second. This helps patients avoid stumbling in the early stages of their rehabilitation, as they relearn how to stabilize their posture. As a result, EksoNR can more effectively mimic proper gait.

EksoNR Certification Training

The full training program consists of two levels lasting 3.5 days and 2.5 days, respectively.

Level 1 (basic): At this stage, physical therapists learn the basic functions of the device and are able to begin therapy with a patient in the exoskeleton under the supervision of another, trained therapist.

Level 2 (Advanced): The next stage aims to familiarize therapists with the advanced features of EksoNR and, indirectly, to increase its clinical application at the facility. During this training, the skills and progress of therapists working with the device are evaluated. Successful completion of the training is marked by the receipt of a certificate authorizing the therapist to work independently with the EksoNR exoskeleton.

There is a 6- to 8-week break between these stages, during which physical therapists have the opportunity to gain experience working with the exoskeleton.

The training is conducted in Polish by a certified Ekso Bionics trainer.
